I would like you to add an EXTERNAL page and an EXTERNAL Form into Wordpress. The added Page and Form should come with WP header and footer as per others WP pages. Your way of doing this should be as simple as possible. You have to write a simple tutorial for this, and then show me how to do this. Both page and form's link should be able to add to the header menu. The form you added should be submitted back to itself and should insert data into MySQL as it worked at the original location.
