//// A necessary introduction //// - I have created a database table that has some patients records, each patient (record) has his civil ID number, file number and his file type. (a Permanent or a Primary care file). - I have installed an appointment booking plugin. - Only patient with "primary care file" who can use the booking plugin, others will be forwarded to a page with a clarifying message. //// what I want //// - I want a search input field inside wordpress page that patient can search database by entering his Civil ID number, if he has a "Primary care file" then he shall be forwarded to URL1, if not, He shall be forwarded to URL2. URL1= an existing booking plugin. (installed already) URL2= an existing Wordpress page. please see attached photos to understand better. I want this script to be work within wordpress pages.

I can achieve your desired functionality ( URL Forwarding conditioned on database status of searched ID ) without breaking or modifying your wordpress code within a couple hours.
