PHP Login Page AND Daily Data Entry Form

Please message me with questions. I need a login AND PHP form built. I need this simple PHP code same day (today). I will build database, etc. on my server. I will build user/vendor/admin registration pages myself. I know how to write code just don't have time. I included details so you can go ahead and write code well. Thank you. PHP needs to be written for form input to be entered into mysql database as follows: HTML form should accompany said PHP code. Step 1. User logs into website with EMAIL and Password only. Not name, etc. Only email/password. Step 2. User is directed to a form (mentioned above) to complete today's yes/no questions. Step 3. User submits form. Step 4. Form data is submitted into the database table called 'data'. Step 5. User's data must include the form data PLUS his userid, vendorid based on his email from step 1. Step 6. User must be able to add new record once and only once daily to database In other words, the PHP gets the userid and vendorid from those tables based on the user's email. Then the user's data record from the form is input into the mysql data table with that. Simple. Database is created as follows: type=mysql name=database1 user=adminconnect (for database connection script in PHP) password=ABcd987!## Tables in database are as follows: admins (admins are associated with vendors...some with few vendors, others with all vendors) users (users are associated with only one vendor and input the data into the data table) vendors (vendors are associated with several admins, several users and data that is input by their users only) data (associated with one user and one vendor only) data will be mostly in format of boolean (true/false or yes/no) and added by each user daily Tables have following fields: admins: id (auto assigned), first name, last name, email, title, region users: id (auto assigned), first name, last name, email, title, vendor, region vendors: id (auto assigned), name, address, city, state, zip, region data: id (auto assigned), date of entry, user, vendor, admin, 30 questions with answer yes/no questions may be labelled as question1, question2, etc. with each having a yes/no/not apply choice please use radio buttons for these choices
