I need a basic form to incorporate into my existing site for members to query my database. I need a form for people to be able to query my database and show the data in a table with options to export it as a pdf or csv. The form needs to have dropdown and radio filter options. I do not need a CMS or a full blow website just a form with the following created in PHP. I have an example screen shot of what I need it to look like and the functions.

Hi, how are you. I have extensive experience working with forms and dynamic querying databases with PHP. Probably the best approach is to create an Ajax based solution using JQuery, creating a dynamic environment. Conversting a MySQL table into csv for export is easy. Then for making a PDF we'd have to get some PHP PDF plugin to convert things automatically from a PHP template.
