There are 2 parts to this job. Create graphically appealing pages feeds 8 fields to php program (already created) and outputs them to a results page. Also create an about-us page. I like the below webiste. Please get ideas about how this page from the below website: [login to view URL] The second part of this job allows user to logon with password and save their data. It display prior entered data and allows them to click on a prior entered row and see the results report for that data. Also allows them to update data. User should also have password recovery option. Also data should be tested and working on my website. The work is programming for hire. And I retain and gain rights to all work done. You must be willing to answer questions about your work. And get the app installed and working on my website. PHP 5.5.30
