For every person that comes in to pawn items they need to have a particular form filled out, signed and thumb-printed (attached). The form has space for the data for up to four items. There are about 10 fields that need to be filled in with prefab codes for each product (see attached form). They do it all by hand now and it’s a pain. He would like to have a system where he can: 1. Log into a website and enter a persons basic contact information and then… 2. print out the attached form with the just the header filled in with the person’s name and contact info. 3. Then the person will sign and fingerprint the page. 4. The pawn guy will then enter in all the remaining data to complete the form IE Items pawned and run the form through the printer again to print out the center area with the product information. 5. He’d like the database to be able to save the person’s contact info and products he’s pawned so he can either reprint the pawn form later or so he can add new products and not have to reenter the person’s contact info. 6. Also keep in mind that it is a requirement that there is only 4 items listed per sheet.. (it is a law) so if the person has 8 items then when he prints that clients items it will print on 2 forms etc. With just that general data, is this something you could program? If so, what ballpark are we in? and time frame? I need this written in PHP MySQL.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered ``work made for hire`` under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
