264158 Zen Cart Customization at chec

I need a script to ask extra questions at checkout in a zen cart store I'm currently building. The checkout is using ESL and FEC combined and the "stacked" option for the checkout page. The first extra question is a text box or area for a gift message. This must appear on the packing slip. Seems like maybe a second comment box tied to the order would do it. The second extra question must be a dropdown menu that I can populate from either admin or phpmyadmin. Naturally, I'd prefer to be able to do it from the admin. The question must be tied to the order; not the customer. I looked at the mod called How Did You Hear About Us, and it looks almost perfect. However, it is tied to the customer, not the order. Seems like a good coder could change that. It's beyond my skill level. (The question is a delivery date request but I'm not looking for a calendar solution. I must specify the delivery windows with a dropdown menu (an entry look like: Early June 2009). I only need to access the options about 20 times each year, so it doesn't have to be slick. I can follow instructions for installation. I have extensive experience modifying osCommerce and some time with my new zen store, but I am not a good coder. The store is not open now. It is vanilla at checkout except for ESL and FEC. It is 1.3.8a. I will be using the classic template and modifying it after all the custom code is in place. I'm hoping this can be done with minimal changes to the core code and with simple implementation of the results in the admin. I'm happy to pay for this but hope that the successful programmer will publish it freely as a zen mod once we're finished. This is not a requirement. I hope I've included enough information for you to get a feel for the project.
