email collector form + database (aplus.net hosting)

I currently host with [login to view URL] and have a Pro account, Unix hosting: [login to view URL] I need to have a simple form built that will collect email addresses as a sort of (subscribe to our e-newsletter by submitting your email address) it will be one siple input with a submit button. The key is that I need to be able to place it on two pages of my current website and those pages are static. I also want the email addresses stored in a database (which is available on my hosting plan) so that I can export them to excel or handle the data with ease later on for mass email campaigns. All you have to do is set up the input form, link it to a new database, and help me place it on two of my site pages (I am a highly trained XHTML CSS experienced webmaster so that should be easy, I jus don't know forms very well). This form should also be secure and not display our email address to the public because I don't want it to be hacked or spammed. I would also like to be able to send one letter to a sort of redirect that would then send that email to my entire database of submitted emails.
