I will use Mailmerge function in Word to do your work. This kind of job is quite easy to me since I sometimes have to do the same work when sending invitation to the players (I am working for a national sport association). I just have to make a dynamic form in Word, then using Mailmerge function and an Excel list of players' information (name, adress, phone number) to create tens of (sometimes even hundreds of) labels for sticking on the envelops.
