I need to create an online form for ordering business cards. The form will have fields such as "name" and "email" for the business card. Once the visitor enters this info, they click a preview button which should generate a pdf which contains the template of the card so they can see what the card will actually look like, with their info filled into the right places. If they are not happy with the results they can change their info in the form, then regenerate the pdf. See attached sample card template, the ones in the left column. There will be a choice of about 6 cards- the difference in card template is mostly the logo The form will also have other fields which will not show up on the pdf. When the form gets submitted, all data should get emailed to the admin along with the pdf attachment of the card (with the person's contact data inside it) I can provide the front-end code of form, so I only need you to code it "work". Also if you can think of a better way of working out this form, let me know.

I have the skill to complete this task. This will be coded using PHP and needs to save the form under a database in case the email fails. Also, this needs a SMTP email account to avoid spam problems.
