Mockup an HTML questionnaire - desktop/mobile - responsive design

This is the design (mockup) of a question/answer system that must be: * SIMPLE - easy to use, and big fonts for older users * Beautiful - nice colours and animations/transitions * Interesting - keep the user entertained * "Happy" - professional but not dry/boring The mockup/prototype is to choose colour themes, fonts, transitions/animations and layouts for a slide-show of questions (questionnaire). After this competition: once the user interface is chosen, the winner(s) will be invited to continue the work for a project to apply this theme to another 40-50 questions in the questionnaire. The questionnaire content will be about medical questions. For the mockup we are using 'pets/animals' questions. Use colours (no imagery) or neutral imagery (e.g. nature, people etc). No imagery is fine for now, we can add imagery between questions, or in the background later. Easy to see, for older people (big fonts etc) * Works on Desktop & mobile browsers (iPad, Android, PC, Mac) - IE7+, Chrome, Firefox, Safari * Fun use of transitions and animations to make interesting Early mockup can be drawings (e.g. pencil, photoshop, illustrator etc), but final result must be HTML for desktop/mobile.
