Web site to hold campaigns embedded to Facebook

Need a website to hold campaign(s) embedded to Facebook "Like" button. Basically, a simple 1 or 2 page(s) website which administrator can change the background image which is embedded to FB. With the above functions for adminstrator to create a campaign. Once created, user is able to "Like" and enter neccessary info create and require by the adminstrator Requirements: 1) Campaign(s) not launch/hold simutanously. 2) Able to create/delete input fields. ie. number of text boxes (able to customize no. of characters, alpha-numeric or numeric) to prompt user when not input correctly. create/delete check boxes if needed. (Administrator) 3) Able to let user refer friends to join the campaign. 4) Able to store the above information in a database including whether user is a referrer or referee or none for each campaign, user's FB account and the date and time of registering the campaign. 5) Able to export the database to excel. (Administrator) 6) Able to recall user info when needed for user to update particulars. 7) Able to check whether user has register for the particular campaign before during user registration. (Check within the Database and not whether user ever "Like" on FB page. 8) Able to change background image for different campaigns. (Administrator) A simple flow diagram will be provided on how the flows works. Once the decision on whoever will be working on this. You are require to set: 1) An instructions guide step by step to guide adminstrator on how to set this up, including how to embed it on FB. 2) Submit all source code with very detail comments on all functions.
