MUST BE COMPLETED IN 5 DAYS. PROFESSIONAL CUSTOM LOOK REQUIRED. SPORTS PERFORMANCE THEME My current site is Wordpress. I am ok with Wordpress or Custom solution. I am looking for somebody who can build an assessment tool that has 40 multiple choice questions (A,B,C,D,E) and based on the surveyor's answers, will provide a custom analysis. Each question has a category and subcategory assigned to it. Each multiple choice (A,B,C,D,E) has a point value assigned to it. I provide the analysis text for each answer. The analysis section must have beautiful graphs/charts to accompany scoring (I will provide formulas). I need you to create a PROFESSIONAL looking survey/assessment tool with the following sections: - A cover page for sign up (I'll provide the text) - The multiple choice questions - An Analysis section (that includes an overall score, score by category, and score by subcategory section). - Recommendations section (which will include the analysis text for questions the surveyor answered C,D, or E for) That's IT!
