I want the format to come to the email in a pdf format, here are the links on how to set that up. [login to view URL] If you go to this page [login to view URL] and click on the General Liabilaty Application it is already set up with Contact 7 forms but not as to come through as a pdf, the top 2 will need set up to look like that layout and I am sending the 2 forms with this so you can see them.

Hi i read your requirements carefully. you need to convert your html contact form message to a pdf format. Also you needs to attached other pdf file if needed. I can use dom pdf php library for this and also used it before. Thank you sumon
