Multi steps form require

I am looking for a developer, who can develope a survey form. It should be a "multiple steps" form. How it should work: Once the user fills all the data and complete the form, the results would be showed. The results would be based on Points that the user would gain by filling the survey form. The Pai Chart diagram shows how many point that a user gain and with the help of the gained points, which service should we recommend him/her. There must be a posibility that I can create, delete and edit form using backend. The form should create an Iframe code, which could use on multiple website pages. The form should be responsive! Php knowledge is required.
