Develop or modify a word-press contact form plug in

This project requires building a new contact form plug in. It will work like this: (Pictures are attached for explaining the process better) 1- The contact form is visible but grayed out (see NDA overlay step 1 .jpg). There are 2 button. "No" and "Yes" 2- If clicked "no" the gray area is removed and the contact form is visible 3- If clicked "yes" an NDA form is opened and the user can fill in information (NDA overlay step [login to view URL]) * The NDA form is uploaded in the admin area as a doc file. Where the text is "$firstname" the document will show an input box for the first name. * Information such as name, subject, date and so on inserted into input boxes within the text of the NDA agreement. This will be saved as a PDF later on. * Once finished, the user fills in his email address and clicks "submit" * A PDF of the document is made and emailed to the user right away. The email contains the PDF document and a link back to contact form step 3. 4- Step 3 - a normal contact form which the admin can design in the back end. * After the contact form is submitted, and email is sent to the user and the admin with the information submitted in the form. * Contact form should allow attaching a file. 5- If the admin defines a payment, the last step shows a checkout step (NDA overlay [login to view URL]) *if payment is not defined as required in the admin area, this step does not exist. * If the user sent the contact form on step 3, a link back to the checkout is also provided, so he can go back to the last step directly without starting from the beginning. * If user already paid, the link shows payment confirmation. if not, the link shows checkout option. (NDA overlay [login to view URL]) * Use simple strip checkout for the payment.
