Develop a Wordpress module

Need to develop a Wordpress module that would allow petition-like functionality. The visitor should be able to express support of the web site content by filling out a form. The list of supporters will be shown in the designated place on the web site. Specifically, the requirement is the following: 1. The civic movement web site contains its strategic program. Web site visitor reads the program and at the bottom sees the invitation to 'sign' it (hence becoming a 'supporter'). 2. First, the supporter needs to authenticates himself/herself through Facebook. 3. Next, the supporter sees a form, which is pre-filled with Facebook details, which can be editable. The visitor fills out the form. 5. The form content is written to the database. 4. Email address is verified by clicking the link in the verification email. 5. Once the email is verified, a notification is emailed to the administrator. 6. The administrator verifies the identity of the supporter and moderates the submission. 7. Once moderated, supporter's details (not all) are displayed in the list of supporters under the form or on a separate web page. 8. Administrator can see the complete list of all supporters with all details in the admin section of the plugin and mass-mail them as necessary. 9. Administrator can export the list of supporters into an excel or csv file. The form will contain the following fields: * First name (text, required) * Last name (text, required) * Email (email, required) * Province (drop-down list) * Region (text, optional) * City/town (text, required) * Organization (text, optional) * Position (text, optional) * How can you support us? (text area, optional) * Additional information (text area, optional) The list of the supporters will contain only selected fields: * First name * Last name * Province * Region * City/town * Organization (text, optional)
