I have been programming for more than 25 years with my primary focus on web based solutions with PHP, MySQL, HTML and Javascript. Based on your description I do have a couple questions about the project: 1) You indicate all questions must be displayed on the same page, do you mean one after the other like the samples you provided, or would you like them displayed one at a time without the page refreshing? Both can be done, but your answer would determine how I create the page. 2) How will the quizzes be scored? Will it be based on the number of correct answers or accumulated answers, such a+b+c = X and c+d+e = Y where every combination of answers has a specific response (like the example you provided) 3) Will all questions all have picture answers like the example you provide or would you like the ability to have a combination of word answers or picture answers? 4) What version of PHP/MySQL will you be running the script on? My bid includes the creation of the quiz script, database and a basic admin system to manage the quizzes.
