4 page online test with extra maintenance window and logon ability

Simple online test with extra maintence windows. User fills in form. Answers 21 questions Resembles the test at [login to view URL] If user skips answer then error should appear at missed answer and they cannot proceed until it's entered. This holds for all questions EXCEPT for answers on page #3. Test has 4 pages. Mostly radiobuttons, select from list and some textfield and textarea. On page 3 user has fill-in-the-blank form but can optionally select from rows retrieved from db. On page 3 there are 2 buttons that pull data from db and lets users select a row Also that page is constructed based on answers from page 2. Pages have already been designed (for look, feel and colors). the coded parts just need to be added. You get a spreadsheet containing table data. so you create the db and populate it (there's very little data - no more then 30 rows of data per table).
