Profile Survey

This system is a website used for inviting users to answer a questionnaire, and letting users to login, see the questions and answer them. After that, each user will receive a PDF file with the answers. ADMINISTRATOR CAN DO THIS 1. Can create users in the system. User must get a link to the website for confirming the login and changing the password. 2. Can invite user to start the questionnaire. Administrator can invite many times. 3. User has demografic data and an email account. Adminsitrator or User enters language of the test and analysis. USER CAN DO THIS 1. User must login and get a questionnaire of more than 150 questions. All questions are multiple choice. 2. User can only go forward in the questionnaire, cannot see previous answers. 3. User must answer the whole questionnaire in one session, cannot interrupt questionnaire and continue later. 4. A progress bar must be displayed at all times "34/150", "35/150", etc. 5. When the last question is answered there must be a message saying "Thank you for answering the questionnaire, the responses will be analyzed and an email will be sent to you to ...@......com (user's email address). 6. A PDF will be sent to user with the analysis of the answers. The analysis is going to be produced by the website. 7. The PDf will also be displayed in HTML form, using a unique URL that will be sent to the user. The name of this URL will be built using the unique user name. GENERAL FUNCTIONS 1. The whole website mut be multingual, both for left to right languages and right to left languages. 2. All strings, messages, all text must come from an Excel spreadsheet with these columns: A. English B. Spanish C. French, etc. Column A (English) must be filled by developer. The rest of the columns will be filled by me. 3. All questions will be stored in an Excel spreadsheet with these columns: A. Question text B. First Option C. Second option. 4. Questionnaire must stop at the last line of spreadsheet. 5. Software mut be developed in PHP, mySQL database, and stored in Amazon Cloud. 6. Answers for a specific test must be stored in the database. 7. When sending results, system must retrieve answers and build a PDF containing: A. User name and demographic data. B. All questions and selected answer.
