PHP/MySQL Existing Script Updates

Take current Word doc or PDF forms and convert them into template that will pre populate with information collected earlier in the script. Customer will have the ability to make modifications where needed. Once the script has all the needed information, the form can be exported out as a PDF that will contain all needed information. Some forms may have fields that do not yet exist in the script and will require additions to the current script with regards to the MySQL database and the PHP script itself if the information being collected is not yet present. Please send questions concerning this project. I am aware that this may not be the best description, but there are three such forms to convert and implement into this script.
