Convert a long form into a wizard system

This project is to convert a 5 pages official form into an easy to use web wizard system. Each user must create an account must meet (email activation+security image) and when they login they can start filling out the form by starting a step by step wizard. The wizard should be extremely professional colorful and very easy to use it should also explain and provide help for each step (layers..no pop ups) A progress bar should appear on the top indicating the status for each step. The user should have the option to save the form and logout at anytime. Security for the data is very important and all sensetive data should be encrypted in the database. Also at any point the user should have the option to export the form as PDF (it should look the same as the original attached PDF file see attachement) minor adjustments might get added. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
