Creating a Dental Treatment Plan Estimator (Designed in Excel to be migrated to Web)

Project Description: I would like to convert my current projects that I have created in Excel, and create a web app version. A version that will streamline functionality and create a user experience that would not require teaching Excel formulas, or knowing how to do certain functions. I just want it to work intuitively and seamlessly. I created the functionality within Excel and have created a mock diagram of how I would like it designed as a web app. It was designed in photoshop so you can review the sections in PSD format. You can review the design attached. One thing to note, this will be more like a Patient Relationship Manager in the end. It will be used as a tool to follow up with patients regarding their treatment and let them know what phase they are in and what will be happening next. It will be used as a tool for administrative personnel so they can easily know where patients are in their treatment lifecycle and who should be coming into the office for their next step. I've attempted to make a small Treatment Tracking tool as well which you will see in the Excel file I also included. I would like for the printed treatment plan document to look exactly as it looks like when it will be printed in Excel. The person who I take to do the job must be well versed in database systems as well as interfacing with MS SQL to pull and write data. Later on I will want to create an interface to pull data from our current system so it does not require us to manually input certain demographic/insurance information. Must speak/write proper english so that we may communicate properly as well.
