Looking to add an appointment booking form on shopify that will work to book eye test appointments for multiple locations. Customer should be able to select city, and then specific location within city, and then choose booking date and time. The same date and time slots can be chosen by multiple people. The form once filled should send a notification via email to my internal email and a nice notification to the customer email. The customer notification email should be similar to my shopify notifications template. With same color and design - for this i can easily provide you with the coding for the shopify templates. And just simple wording saying thanks for your booking, here is the date and time and particular location, look forward to serving you then.

Hi, requored appointment form with slectable location/city, date etc will be added to your shopify store, and email feature to send email with similar template as shopify email template will be added (it may require external handling, so we may have to test it with possible options), the form and mail feature doable in 3 to 4 hours, let me know if you are interested, thank you
