CKEDITOR Form + Image painting + Math + -- 2

I want a plugin to Ckeditor that allows me to create: - An equation creator to apply in the form elements of the ckeditor , this maths wil be previously created. for example: i will add some form inputs, then these inputs will be loaded into the plugin , inside the plugin the user will be able to create a math expression. this code is partialy created, the old freelancer has gone and deleted the account, i woul like some one to finish it. Images in the description. What is missing is: - it is creating the equations , but it is not getting the name of the inputs to create it, and its not loading the equation after creation , if you delete it it doesnt show the inputs again
