Acrobat PDF Form creation and design

I want someone to create and design a dynamic PDF form. I have attached an outline spec showing what needs to be in the form. It is a form for a customer to complete to incorporate a new UK company. As such it will need to change depending on what the customer wants (ie if they want multiple shareholders then the form will need to add more pages for details of the shareholders). It should be capable of being filled in using acrobat reader and then a submission by email button will be required. It is to be a practical form and no fancy design is required, but it does need to be clean, professional looking and easy for the customer to read and understand. I have used a simple Word table to set out the spec, but I would not expect the final form to look like this. I will provide my logo for inclusion on the form as well. When quoting for this job, please provide examples of other forms that you have done (not just generic work) and confirm that the spec can be achieved. Many thanks Nick
