I would be happy to insert a form into your site. I will require the source code for any of the pages you want the forms added to and a description and list of what you need. I will post the code to Github in a new project so that you can easily track the development and retrieve the code when it is complete. This is a fairly simple project as web forms are supported by web browsers by default.
