I manage an HR outsource company for long term care facilities. One of the things we do is obtain references for employees. i would like a simple site that has an attached database that can do the following: 1) Employees can input basic info on new applicants; 2) track teh reference gathering process as it progresses (i.e. phone or email attempt, person spoken to, etc); 3) Export this info based on request; 4) Allow a form to be emailed or generated through using Jotform or similiar; 5) Create a PDF form for any applicant of the above information

Hi, I create simple wordpress-based (with twitter bootstrap ui/ux) sites a lot. I usually then create custom applications (using plugins) for clients. What you've describe in your project seems to fit my skills and I'd be interested if you'd like to chat about further details, in case anything is left out of your project description. My usual first step is to create a small statement of work (SOW) that makes sure I understand what you need. I have my own server and as such, if needed, I can host and maintain your site/application over the medium term, if that might work out well for you.
