Check form for simply accounting enterprise 2009

I need to develop a check form for Simply Accounting Enterprise 2008 using Crystal reports 9. The check form needs to be on a 7" x 8" page looking like the attached file. All the writing in blue will be fields from the Simply accounting Database. I need this ASAP and I need a tutorial to learn how to run it from my end. The project will be complete only when the form is being used successfully on my end.
