Copy Survery Style - repost

- Make a survery as the one sended to you. that saves data in mysql. U can asnwer it onece per ip, and have one link as the one i sended ([login to view URL] or [login to view URL]), when u asnwer it the link do not change. - Survery must have a specific logo, back, and color of the rectangle with mini round corners. - Must have different types of ways of answerings as the example, but especific: type of questions yes, 3 types of questions, yes, no, non N/A, options (one or many), and exploratory that u can write text, like open qustions. This are the posibilities. posibilities. - 2 Languages (English and Spanish), i will give in spanish, u stranslate with trasnlator. - must support 3.000 to 5.000 surverys in total in a month - Button t download data when they want, and download last data update of msql. ofcourse this button can be in the survery, must be in another page. Data, must be download in excel in a specific button. - Must be seen in pc or mobile, must adapt to any device. - 20 questions aprox
