Custom Assessment Instrument

I have designed an assessment questionnaire to be used as an intake qualifier for prospective clients to my coaching services. It utilizes open-ended, rating, multiple choice, and true/false questions. My intention is that once the respondent has answered the questionnaire, he can be rated as to his readiness for my services, as well as readiness to engage in new relationships ... all according to pre-determined response criteria I have selected. The results the respondent gets will advise him as to which group he fits into ...all to be handled by autoresponder. I am in need of a program to handle this assessment, and I intend to keep the cost of developing it to t minimum. ## Deliverables At the beginning of the assessment there needs to be space for some introductory instructions. Also, the subsequent questions will be grouped into sub-groups, so there will need to be a way to ented sub-group headers throughout the assessment.
