Additional customer info enhancement, for value4u only

Hello Value4u Please read the project description, I'm sure this will be very simple for you. 1.I need customer to be able to enter some more information while creating a new account By default, they will enter Gender, Date of Birth, Company, Suburb, State, but now I hope they can enter below information: Height/Weight/Chest/Waist/Hips/Inseam/Outseam/Shoulder Width/Back Length. They can leave it blank if they are not sure, so these information should not be required. I hope there will be two prepared answers underneath with check box. One is 'Myself' and the other is 'For others'. [login to view URL] the attached contribution, but I want you do some modification on Order Edit Page. The contribution will show customer#, Order #, Date & Time#, Shipping Method#, Payment Method. I hope we can see below information: Payment Method,Customer #, Order #, Date of Birth, Gender, Order Placed as a gift for: Myself/for others Height/Weight/Chest/Waist/Hips/Inseam/Outseam/Shoulder Width/Back Length. For you information, the sql file has been already imported I hope you can finish it within today.
