Create a PHP / JQuery Booking Form(repost)

Hello I need a booking form created. I want it in the format of: <[login to view URL]> This is a JQuery Tools addon. You are to create the form and ensure all is functional, you will be given an old booking form and a new database. You will WORK on your OWN server! The Booking format will be the following detailed in the deliverables: **Finally, this is a simple project and requires you to just order the forms and make it look nice and professional, Lowest bidder will win with high ratings. Thanks** ## Deliverables The following is how it should be laid out - This should be fully jquery / ajax and calls for values from external php files. I should be able to call this function on any page - basically i will have a button (link in the link) and it will open this form. **Section 1:** Item (Selection box of all products) Date (Use the calendar, that is in the script above) ** When the user complete this section it will check if the project is AVAILABLE for that date, using jquery ajax to load the php script, if not available then it will sate sorry this item is NOT available for this date. If is available it will proceed to section 2. ****Section** 2:** Type (Selection box - just fake stuff, i will fill this in) Overnight (Radio Buttons - Yes or No) Additional Information (Textarea) ****Section** 3: ** Title (Selection box - Mr, Mrs, Miss etc) First Name (Text field) Surname (Text field) Address Line 1 (Text field) Address Line 2 (Text field) Town (Text field) County (Text field) Postcode 1 (Selection box of all postcodes/delivery in database) Postcode 2 (Text field) Telephone Number (Text field) Email Address (Text field) ****Section** 4: ** Here will show the prices. Item Price: Overnight: (If ticked yes shown, else not shown) Delievery: (Price is taken from postcodes/delivery) Total Price: All above added up, ensure there is a 2 decimal place and all prices displayed as Pounds ie ?10.90 Payment Options (Selection box - Cash on Delivery, Paypal) If Cash on Delivery is selected proceeds to page 5 If Paypal is selected a text field will display below the selection box. Paypal Email Address (Text field - No functionality required, ie submitting the data to paypal, i just want the text field to show) ****Section** 5: ** Confirmation of all of the above, with each page in sections, with a small "edit" button so they can edit any part they require. Capcha Required here when submitting the completed form (same as the old booking form). The data is then saved in MySQL and sends an email confirmation to both the client and the admin (same as the old booking form). **Design:** Should be similar to the preview given above. The other pages like The Code, User Experience etc is NOT required. I should be able to change all the background images, the button images and font via CSS. -------------------------------------
