customer service trouble ticket system

This project consists of a trouble ticket system for a service business unit in charge of resolving hardware problems on customer industrial equipment. The trouble ticket system will be based on a mySQL/PHP database and hosted on an intranet web Apache server. Access will be login dependent. Currently all data concerning customer and equipment installed base is available in excel files. This data will have to be imported from excel files into new mySQL database. There will be different levels of users: administrator, technician, supervisor. The different level of users will have different access rights. Administrator can assign levels to users. The ticket form will have almost the same look as the paper form currenlty used. There will be views, whose access is user dependent, to view, edit and export the list of the customers, equipments and tickets. To simplify future maintenance it is required to put all HTML, PHP and images files in directories named after the chapters contained in this specification. For example all code related to the login should be in a directory called “1_Login page??, similarly those related to the Main menu in “2_Main Menu?? and so on. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Linux, Apache, MySQL, PHP, Browser for testing: Internet Explorer
