Database Admin Pages

I have a system where customers place requests to the system, providing certain data, and then the system calculates a set of results for them. I require a set of admin screens written in PHP and HTML to allow me to: 1) View Customer Info (given cust_id or name or request_id) 2) View Request Info (given cust_id - linked from (1), or request_id) 3) View Input data usage for a given request 4) View Result information for a given request Suggest some type of master-child (or tree like) structure - one customer has 0 or more requests which has 1 input data (of 1 of 2 possible types) set and 1 results data set. Additionally, I need to be able to create/update/delete a Customer, Request and usage information. It does not have to be visually brilliant, but functionally efficient. Attached is a screenshot of the current Access DB - showing a search screen with customer summary block and below a tabbed block showing request, usage, result information. First you search for the customer which is populated in the list to the right and then select the customer record which populates the bottom screens. Please see attached documents for more information. Note, that I supply the DBSchema to the selected bidder and can also if necessary supply the required SQL. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Needs to run with MySQL 4 or later using recent PHP and must run on at least Internet Explorer.
