Build a basic but responsive form in webpage with an admin panel. There should also be a 'Why do we need this' section which I can edit from the Admin Panel. The form fields: Show Name: (From a drop down fed from the admin panel) Name 1: DoB: Email: Name 2: DoB: Name 3: DoB: Name 4: DoB: Name 5: DoB: Name 6: DoB: Submit. Then a confirmation message pops up. From the Admin Panel I must be able to select if I want DoB to appear or not. And I must be able to change the Maximum amount of names. (Some shows will be 2, others will be 6. On the admin panel I must be able to export as .CSV depending on Show Name. Each show must also have a Image, that is displayed when the show is selected. This task needs to be complete TODAY (within 20 hours)
