Create a fully functioning health & well being website: one of the main site functions will be: Interactive form with series of questions. Will contain mixture of drop down boxes, yes no buttons, free text etc. if possible autocomplete (selectable options change colour on mouse over see example picture) form will have possibly a breadcrumb approach at top of form to show user what stage they are in the process once submitted these results will go to a database with option for user to print results & also email themselves. everytime a form is submitted a email notification will be generated to admin. . Other items symtom checker Pick me up intuitive contact us form (Similar to attached pic) Add twitter feed box add social media icons/links Able to add shares on mobile devices for Whatsapp, twitter & FB Want to be able to add/remove questions easily. Add/remove pages easily Has to be mobile friendly
