Asp.net 2.0 Online Form Builder

We require an online form builder which allows users to create forms such as registration forms, surveys, email subscriptions forms etc and capture repsponses in a database.

We are only looking for the ability to create the form and store the results in a database.

We do not need to be able to create user accounts, seperate forms per user etc, and any of the other advanced features these sites offer.

We will only consider bidders who can demonstrate strong experience with the following technologies.

We will finalise a requirements set once we have chosen a qualified bidder.

Please indicate a starting bid, this may be revised once we have finalised the requirements.

The devliderables will be vs 2005 solution with a clear n-tier architecture, seperating UI, buisness logic and data access.

If you have not used all the technologies above but are confident that you can deliver the spec we require, an outline of how you would approach the project would include you for consideration.

Bids which do not include samples or a clear outline of how you can deliver what we are looking for will not be considedred.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
