contact - help desk manager

I want a PHP / MYSQL web site for my contact management. I don’t need a great deal and I have over half of the thing written already. I just don’t have the expertise to finish it.

I need name, address, phone number. Room for 2 more phone numbers and 3 email addresses.. A website holder I can click on to link me to their site. I need a link to notes for the record. The index will be on the phone numbers. The notes will have the date imbedded and retrievable by date or customer (phone numbers).

I want to see the format in the [url removed, login to view] as follows

Company first last phone web site link email links.

Company first last phone web site link email links.

Company first last phone web site link email links.

Etc.

The phones & email are clickable links.
