Update existing contact form

The form at <[url removed, login to view]> currently is not functioning. The code was written several years ago for a windows server platform and it was done using the frontpage editor. The page has since been moved to a linux platform server and now the code needs to be rewritten to function properly. All display, form fields, fonts are to remain as they are. The output of the information typed into the form is to be sent to [casalvatori@[url removed, login to view]][1] .

Also I want a simple captcha added to the form to be active to allow form submission.

Pls include java script to display the message -

Thank you,**Contact_FirstName** for your interest in Carlos Salvatori Consultants. We will respond to you as soon as possible.

( where contact_firstname is the input value for that field input into the form)

This message should appear to the user after he submits the form.

I will supply access information to the server when job is accepted.
