Create a form for Sharepoint

To develop a form for project managers to check off and auto populate the date and add any comments, if it can route to another party within sharepoint that would be great but it would need a trigger when the level is code orange or red.? I have a? temlate form that needs to have a status selection and i was hoping to use the three circle icons in the COnfditional formatting as an option but can't get it to work. I want the form to look document like and not spreadsheet in appearance.? I also needthis by? Friday, July 31, 2010.? Thank you.
