Very simple site

I need a webiste one landing page and another page which is a form .

i need it for wednesday to go live .

when the form fill i want it to go to my emails and also go to the mailchimp

i can do this myself but am so busy so need someone to do it

i will provide the design

i dont mind if its basic html design or wordpress etc .
