I have a new website I am launching for a product. I need someone to put a basic banner on the top of the page and then have 1 page (it will be the home page for now) that will be very basic. I will have some copy and I need a comment box created as well as 2 smaller boxes that correlate with the larger box (the 2 smaller boxes are just name and email). I will need the submit button to route the comments, name and email to my email address (but obviously do not want to show my email. I have the copy and a design example. (banner will be provided)
