Bespoke Wordpress App - quote system

I am looking for someone to build me a plugin or page that allows my clients to select options they want which then calculates the total based on what they have selected, a bespoke quote system.

I have attached a sample spreadsheet to demonstate what I need. The cells in green are the fields my clients will tick (i would like a tick box) based on their requirements.

The page much be customizable so that in future if i want to add new items I can and also if I want to change my price structure.

Finally a submit button that emails the quote to them and myself. The whole thing must be installed directly onto my website
