Development of a solution to "daily schedule" and "electronic medical record" for health professionals

We need to hire, for integration with a system that already exists, the development of a solution to record regular consultations in the "daily schedule" of health professionals and to allow the scheduling of emergency consultations (fittings).
The scheduling module must have day schedules of view (daily schedule), weekly and monthly. The scheduling module must display subtitles to report that the query was confirmed, the patient is in the waiting room, the patient was treated and was generated a tab for the service.
We also hire the development of an electronic medical record module that allows registering and querying history in medical records of patients, printing receipts, medical referrals and testimonials.
It should be developed also a registration module and issue of registration of patients.
Development must be done in PHP5 with PDO and the codeigniter framework must be used (using MVC). The data reported on the forms of the fields must be recorded in a MySQL database that is already being used by other system modules.
The system must have functionality of sending SMS messages to patients and the schedule should allow integration with the agendas of smartphones.
The layou to be used in the modules is already set and we have all system screens specified.
