We have 250 domains that require such development and we are willing to pay up to $100 for each page created.

We require not only the design but basic PHP also in order to store leads in a database and also redirect visitors to another site once we have capture their details.

Eventually we will want to move all our sites to be managed under one admin portal.

For now, I have attached a banner for an example Affiliate Link that we would want to report.

Design a Simple, but professional looking page around that banner. With a Form that captures the following information:

Name

Amount To Borrow

Email Address

Password

Mobile Phone

Age

CAPTCHA

This information will be stored in a MySQL database along with date/time, referrer and IP address.

The Landing Page Name For this Purpose Will be ONE CLICK LOAN

I appreciate that the description is vague, what we are looking for is a partner who will work with us long term creating up to 3 landing pages for each domain name.
