and check ALL: HIV Tests, STD Tests, and STD Vaccines
For example, zip code: 75035 and search radius 35 miles. Checked all boxes below. This returns 36 results.
I need the following information for every zip code in the United States:
1. Clinic name
2. Address
3. Phone
4. Clinic type and notes at the bottom of each listing
5. All the services that are offered at each clinic (in the bullet points on the right side)
I attached a sample excel file with the entries that are needed. I will need this data for every clinic in the United States. I will need a csv and excel file if possible.
I will chose the person that can do the work quickly and for a good price. I am wanting to stay under $100 if possible.
