Client Website Needs Staff Login; Form Sub to PDF Population

Existing client website with custom CMS already in place needs further work.
Project consists of top level admin creating staff login accounts. Staff can then create third party login accounts for website. Third parties will be able to submit form data that is not only viewable and exportable to CSV file, but will autopopulate PDF forms (provided).
Third party login pages will also have other functionality outlined in the attached document. One of the sample pdf files has also been attached for you to see. Further details will be provided upon discussion.
Development time should take no more than 1 week for any experienced developer.
If you are chosen as the bidder, and you a competitively priced, we will consider using you for all further projects (we are looking to replace current staff programmers; currently sitting on 3 other projects waiting to begin). WE DO NOT TOLERATE MISSED DEADLINES. Please provide samples of your work. Also, we DO NOT work with wordpress as our clients expect OUR CUSTOM CMS design and functionality. If you are only a wordpress developer, DO NOT APPLY.
