Comedy Flyer Design Needed

I need a flyer designed to be used to advertise my comedy shows.
Flyer must be at least 300 dpi for high resolution and will be printed in sizes of 8 1/2 X 11 and 11 X 17.
Here is the outline and the flyer must contain the following words below
Comedian "MG"
Performing Live (must leave a blank space big enough for someone to write the name of the club, the date, and time of performance...see example)
As heard on Sirius Satellite Radio and the Rick and Bubba Show (you can get logo from [login to view URL] and [login to view URL])
Flyer must say this
Featuring a Live Q and A session with "George Bush"

I will not pay more than 30 dollars to complete this project due to the simplicity of this flyer. Make sure when you bid you include specific examples of your work or even an example of the flyer I have described above.
Use your imagination and be creative. I have included files below. One is an example of the flyer and the other two files are headshots you can use of me in the flyer (File is too big to send but included one to use but will email exact one to winning bidder)
Final project to be sent in JPEG and PDF file.
