Design an App Mockup -- 2

Design an app or a link that can be emailed or downloaded to our potential customers. It will guide the owner step by step thru the process of filling in the details we require to estimate and provide a full and complete quote for repairing their accident damaged vehicle. Details required : Name, address, contact details ,mobile phone numbers, email address, Insurance company, Claim number, Vehicle registration number, Vehicle chassis numbers and built date, Images of vehicle options, Images of vehicle damage, comments, .....

An app or program that will guide them thru the process step by step and provide us all the information required to quote the vehicle along with providing us the images of damage, compliance plates, registration plates, speedo, and any other relevant required information.

Must be an easy to use, simple program for capturing information in a step by step guided process.
